Note, not all fields are required.  The following fields must be completed on the form referenced above: 
"Situs Address"
"Owner Name(s)"
"Owner Address"
"Owner's Estimate of Fair Market Value as of January 1st - Taxpayer Assessment (TPA)"
"Home Phone"
"Work Phone"
"Taxpayer Signature" and "Date"
 

Form must be filed by mail or in person with the Fulton County Board of Tax Assessors by April 1, 2010
If you are mailing the form, it is suggested that you use CERTIFIED MAIL. Your return receipt will serve as proof of delivery.
The Property Tax Return/TPA must be received in the tax assessors office or postmarked by the U.S. Postal Service on or before April 1.
